Community Health Partnership were developed in order to strengthen partnership working between health and social care and primary and secondary healthcare. There are a number of different structures, here in the Western Isles, the Community Health and Social Care Partnership is an integrated body with accountability to both the Western Isles Health Board (NHSWIs) and Comhairle nan Eilean Siar (CnES).
The partnership incorporates, primary care services (GP's, Dentists, Pharmacists, Optometrists), community nursing services, Allied Health Professions (Dietetics, Occupational Therapy, Physiotherapy, Podiatry and Speech & Language Therapy), Mental Health and Learning Disability services and the community hospitals of Ospadal Uibhist agus Bharraigh , Isle of Benbecula and St Brendan's, Isle of Barra. The partnership also incorporates adult social care services managed by CnES, these include Social Work, Home Care and Care Home services; together with Mental Health and Learning Disability services and Housing. |
